Acceptable Use Agreement Forms 

Student Pledge for COMPUTING DEVICE Use

1. I will take good care of my COMPUTING DEVICE.
2. I will never leave the COMPUTING DEVICE unattended.
3. I will never loan out my COMPUTING DEVICE to other individuals.
4. I will know where my COMPUTING DEVICE is at all times.
5. I will charge my COMPUTING DEVICE's battery daily.
6. I will keep food and beverages away from my COMPUTING DEVICE and other student COMPUTING DEVICES, because they may cause damage to the devices.
7. I will not disassemble any part of the COMPUTING DEVICE or attempt any repairs.
8. I will protect my COMPUTING DEVICE by only carrying it APPROPRIATELY.
9. I will use my COMPUTING DEVICE in ways that are appropriate, meet FHASD expectations and are educational.
10. I will not place decorations (such as stickers, markers, etc.) on the COMPUTING DEVICE. I will not deface the serial number sticker on any COMPUTING DEVICE.
11. I understand that my COMPUTING DEVICE is subject to inspection at any time without notice and remains the property of the FHAS School District.
12. I will follow the policies outlined in the COMPUTING DEVICE Handbook and the Use of Technology Resources Policy while at school, as well as outside the school day.

 

The next 3 items apply to middle school students only.

1. I will file a police report in case of theft, vandalism, and other acts covered by insurance.
2. I will be responsible for all damage or loss caused by neglect or abuse.
3. I agree to return the District COMPUTING DEVICE, case, power cords and any other accessories in good working condition.

I have read and understand the stipulations set forth in the above documents including the COMPUTING DEVICE Policy, Procedures, and Information; the Acceptable Use Policy; and the Student Pledge for COMPUTING DEVICE Use.

My signature below identifies that I have read and understand the Ft. Huachuca’s COMPUTING DEVICE Policy, Procedures, and Information Guide and have discussed this material with my child. 

I agree to the stipulations set forth in the above documents including the COMPUTING DEVICE Policy, Procedures, and Information; the Acceptable Use Policy; and the Student Pledge for COMPUTING DEVICE Use.

Individual school COMPUTING DEVICE and accessories must be returned to the FHASD COMPUTING DEVICE Support Station at the end of each school year. Students who graduate early, withdraw, are suspended or expelled, or terminate enrollment at FHASD for any other reason must return their school COMPUTING DEVICE prior to the date of termination.
